Force your team to dig deeper by asking "why" multiple times until you uncover the structural, process, or people issue causing the breakdown.
Once the root cause is clear, your solution must involve a permanent change. This might mean updating a core process, clarifying a seat on the Accountability Chart, or providing additional training. Document the solution as a clear, actionable weekly To-Do with a single owner and a hard deadline. If the solution requires updating a documented process, make sure it is trained and followed by all. By focusing your solutions on structural changes rather than temporary fixes, you will resolve issues once and for all, keeping your weekly meeting efficient and preparing your business for a clean exit.
