Our Level 10 Issues List has accumulated over fifty unresolved items, creating severe cognitive overload and decision fatigue before we even begin IDS. What is the exact protocol to prune and reset an overgrown Issues List?

An Issues List with fifty or sixty items is not a sign of operational diligence. It is a dumping ground that creates chronic anxiety and decision paralysis. When an Issues List balloons out of control, the leadership team loses sight of what truly threatens enterprise execution.

To reset an overgrown Issues List, schedule a deliberate fifteen-minute cleanup at the start of IDS or during your next Quarterly Planning Session using the following triage protocol:

- Enforce the 90-day expiration test: Review every issue older than thirty days. If an issue has sat untouched on the list for a month and the business has continued to operate profitably, it is rarely a critical priority. Delete it or move it to the long-term Issues List on your V/TO.
- Consolidate common operational roots: Often, ten seemingly separate operational issues are symptoms of a single underlying defect, such as an unclear standard operating procedure or an accountability breakdown. Group related symptoms into one definitive root-cause issue.
- Push tactical tasks to To-Dos: Frequently, minor administrative annoyances get logged as issues when they do not require group problem-solving. If an issue simply requires one person to make a phone call or update a document, assign it immediately as a seven-day To-Do without running it through IDS.
- Delegate to departmental Level 10s: Strip away issues that belong to middle managers and frontline teams. If an issue can be solved inside a departmental meeting, push it down immediately.

Keep your weekly Level 10 Issues List lean, ideally between ten and twenty high-priority items. When the list is manageable, the team attacks the top three issues with urgency rather than feeling overwhelmed by administrative clutter.
