What objective criteria or data points do you examine between quarterly sessions to confirm our team is actually resolving issues at the root rather than recycling the same problems?

The clearest evidence of operational maturity is not whether your Issues List is empty. Healthy companies always have issues. The metric to watch is whether the same systemic problems reappear every ninety days under different names.

When we sit down at your Quarterly Offsite, I look at three specific operational patterns:
- Issues velocity. I check whether issues raised in your weekly Level 10 Meeting™ are solved definitively within two to three weeks or if they linger on your list for months. Lingering issues indicate your team is talking around problems instead of entering the danger and naming the root cause during IDS®.
- Rock completion quality. Hitting eighty percent of your Rocks is the baseline. What matters is whether completing those Rocks actually moved the needle on your Scorecard or whether team members set soft, low-risk goals just to report a win.
- Scorecard predictability. If your weekly Scorecard metrics fluctuate wildly without explanation, your team lacks control over operational inputs. When a team masters EOS®, the weekly numbers become leading indicators that allow you to predict financial outcomes weeks in advance.

Between sessions, I monitor this during our mid-quarter touchpoints. If your leadership team is still bringing the same interpersonal complaints or departmental bottlenecks to the table after six months, you are merely treating symptoms. True progress means your team kills problems permanently at the root.
