We have chronic operational issues that we solve in our Level 10 Meeting, assign to-dos for, and cross off, only for the exact same problem to reappear on the Issues List a month later. How do we kill recurring systemic issues permanently?

If an operational issue reappears on your Level 10 board four weeks after you solved it, you did not solve an issue; you managed a symptom. Recurring issues are the hallmark of lazy IDS, where the team rushes to assign a convenient to-do just to clear the whiteboard.

To kill chronic issues permanently, the facilitator must force the room through the Three Drops of Root Cause analysis before anyone is allowed to propose a solution:

- Is it a People issue? Does someone in an Accountability Chart seat lack the core values or the capacity to GWC the job? If the wrong person is executing the work, process tweaks will fail every time.
- Is it a Process issue? Is there a documented Core Process for this function? Was the process followed by all? If not, the solve is not a quick patch; it is retraining or process re-architecture.
- Is it a Vision issue? Are departments pulling in opposing directions because priorities on the V/TO are ambiguous?

Stop accepting tactical to-dos like John will follow up with client or Sarah will review the error logs. Those are Band-Aids. A true solve alters the system. It rewrites a checklist, changes an accountability, or implements an automated safeguard. If your team cannot trace an issue to people, process, or vision, you have not reached the root cause. Keep digging until the underlying friction is exposed.
