Our leadership team is consistently missing our ninety percent To-Do completion rate target, usually hovering around sixty percent week after week. Everyone has a list of reasons why their tasks are incomplete, but the work is piling up and dragging down our Rocks. How do we break this cycle of low To-Do completion and restore accountability?

A consistent failure to hit the ninety percent To-Do completion rate is a major warning sign of a broken accountability culture. When sixty percent becomes your norm, your team has silently agreed that commitments made in the Level 10 Meeting™ are optional.

To fix this, you must first change how you review To-Dos. During the weekly pulse, the Integrator must ask each owner if their To-Do is done or not done. There is no third option. Do not allow anyone to say it is in progress or almost finished. If it is not one hundred percent complete, it is not done.

Next, when a To-Do is missed for a second consecutive week, it must immediately be dropped to the Issues List. Do not let it sit on the To-Do list as an ongoing failure. Frame it as an issue during IDS®. Ask the owner: Why is this not getting done? Is it a capacity issue, a clarity issue, or a lack of commitment?

By moving incomplete tasks to the Issues List, you force a real conversation about priorities. If a leader lacks the capacity to complete their weekly To-Dos, they may have too many Rocks, or their seat on the Accountability Chart may be overloaded. Stop accepting excuses and start solving the operational bottlenecks.
