Your leaders are using the Headlines section as a shield to avoid the accountability of the Issues List. By framing a major problem as a simple FYI headline, they get to share the bad news without having to defend their performance or solve the root cause during IDS®. This behavior breeds operational complacency.
To stop this, the facilitator must enforce a strict boundary between reporting and solving. The rule is simple: if a headline requires action, discussion, or a decision, it is not a headline. It is an issue.
When a leader shares a headline like a major client threatening to leave, the facilitator must immediately interrupt and ask a single question: do we need to solve this? If the answer is yes, the facilitator must direct the team to drop it down to the Issues List. Do not allow any discussion, explanation, or context in that moment. Move directly to the next headline.
