How do you handle accountability inside the room when a leadership team member repeatedly makes excuses for missing Rocks or operational numbers?

My job in the session room is not to play judge or punish executives. My job is to hold up a clean mirror to the leadership team so reality cannot be obscured by excuses or emotional defenses.

When a leader repeatedly misses Rocks or drops their weekly Scorecard measurables, we do not ignore it, debate personal intent, or accept superficial justifications. Instead, we immediately drop the issue onto the Issues List and process it through IDS®.

We start by separating the person from the seat using the Accountability Chart and the People Analyzer®. We evaluate three root causes:

- Was the Rock poorly defined? A Rock must be specific, measurable, and attainable within ninety days. If the finish line was ambiguous, we correct our scoping discipline.
- Is this a capacity issue or a structural failure? Does the seat have too many competing responsibilities, or were critical resources missing?
- Is this a GWC™ issue? Does the individual truly Get it, Want it, and have the Capacity to deliver in that specific role?

By treating a missed objective as a systemic issue rather than a personal failure, the defensiveness leaves the room. The leadership team evaluates the hard facts together. If the root cause is a lack of capability or commitment, the owner and Integrator are given clear data to make the necessary personnel decision swiftly.
