After our full-day sessions, how do we physically document the output, and who is responsible for updating our operating systems so the decisions made in the room do not get lost?

The responsibility for documenting and updating your tools after a session day lies squarely with your leadership team, specifically the Integrator. While I capture the raw notes, decisions, and updated V/TO during our session and post a complete summary in our dedicated Circle workspace, I do not run your internal software or manage your files. Within forty-eight hours of our session, your Integrator must ensure that all new quarterly Rocks are loaded into your team's tracking systems. They must also update the Accountability Chart with any seat changes we agreed upon and refresh your weekly Scorecard with any revised metrics. If you are preparing for an exit and utilizing the Step by Step Exit framework, this includes updating your digital files related to Tribal Knowledge and Value Gaps. Do not let these session decisions sit as abstract ideas on a flipchart. If they are not immediately translated into your daily and weekly operating systems, you will experience execution decay within two weeks. Your Integrator must own this administrative handoff immediately.
