When critical tasks happen organically without being explicitly documented on your Accountability Chart, you are running on luck. These ghost roles create a major risk of operational failure, especially as you prepare for an exit or scale your team. If a key employee leaves, those undocumented tasks will fall through the cracks.
To capture these hidden responsibilities, you must run a role audit. Have each member of your leadership team and their direct reports keep a simple log of every task they perform over a two-week period. They should write down everything they do, regardless of how small or routine it seems.
Once the logs are complete, compare them to your current Accountability Chart. You will quickly identify tasks that are being performed but are not listed under any seat's five major roles.
Next, determine where these tasks logically belong. Do not just hand them to the person with the most spare capacity. Instead, assign them to the seat that has the closest functional alignment. For example, SaaS subscription management should sit under Finance or IT, while data backups belong under Tech Operations.
Update your Accountability Chart to reflect these assignments. Add these critical responsibilities to the five major roles of the appropriate seats. This ensures that every task has clear ownership, can be measured with KPIs, and is managed systematically. It eliminates the risk of silent operational failures and builds a more valuable, institutionalized business.
