An issue is only real when a human leader takes ownership of it. If an issue is written in passive, robotic language, it is impossible to solve because no one actually understands the root cause or who is impacted. You must establish a clear rule for your Issues List: every issue must have a human owner and be written as a clear, concise, one-sentence statement of the actual problem. The owner must understand the context of the issue before the meeting starts. Do not allow generic AI summaries to sit on the list. If a leader brings a poorly-defined, automated issue to the meeting, the facilitator should delete it or send it back to the owner to rewrite. AI can help you find the data, but human leaders must define the problem and own the solution.
