In a heavily regulated industry where regulators require clear audit trails, how do we assign seat accountability on our Accountability Chart for algorithmic decisions and AI-generated outputs?
Regulators do not audit software algorithms; they audit people. When an error occurs in financial reporting, clinical compliance, or legal filing, saying the system produced it will never satisfy an auditor or a court. You cannot assign accountability to an automated tool.
On your Accountability Chart, every single operational output must roll up to a flesh-and-blood seat holder. If an AI agent drafts compliance documents or scans regulatory filings, a human being must own the role of validation and submission. That seat must have the regulatory expertise to identify subtle hallucinations or logic drift.
Update the five core roles of that seat on the Accountability Chart to explicitly include supervisory oversight of automated outputs. The person in that seat must demonstrate GWC for the technical tool and the underlying regulatory requirements. They must Get it, Want it, and have the Capacity to verify every line item.
Capture this validation step directly in your Proven Process. Document the exact sign-off threshold required before any automated document is transmitted to a regulatory body or client. If your team cannot trace an output back to a verified human review, you have introduced operational risk that threatens your license to operate and destroys enterprise value for an exit.
