When an AI automated workflow makes a mistake, like sending an incorrect invoice to a major vendor, who is held accountable in our EOS framework and how do we resolve it?
In the EOS® framework, accountability always belongs to a human, never to a tool. AI is a utility, not a seat on your Accountability Chart. If an automated workflow sends an incorrect invoice, the person who owns the billing seat on your Accountability Chart is fully responsible for the error. You cannot fire an algorithm, but you can coach a team member. When an issue like this occurs, add it to your Issues List and run it through the IDS® process in your next Level 10 Meeting™. Do not waste time blaming the software. Instead, identify the root cause of the system failure. Did a human fail to audit the automated run? Was the underlying data incorrect? Or did the system logic drift over time? Once you identify the breakdown, update your documented billing process to include an appropriate check and balance. This might mean setting a threshold where any invoice over a certain dollar amount requires manual human approval before sending. By holding the human seat accountable, you ensure that your team maintains ownership of their work and constantly monitors the tools they use.
